As part of better long-term-condition management, a remote end-to-end digital pathway is offered to West Suffolk patients who may be at higher risk of atrial fibrillation without knowing it.
Objective
To provide more joined-up care and better long-term-condition management by finding atrial fibrillation earlier in patients at higher risk.
Approach
After risk stratification and an eligibility check, patients receive an automated text message to download FibriCheck and activate a 7-day code. They measure their heart rhythm twice a day for a week, and whenever they experience symptoms. The measurements are reviewed by the cardiology team at West Suffolk Hospital.
Outcome
Where further investigation is required, patients use an ECG patch to confirm FibriCheck’s findings and help establish a diagnosis. Once a diagnosis is established, patients are contacted by a specialist clinical team to discuss treatment where applicable. The result is a single remote pathway that runs from risk identification through detection, diagnosis and care.
