How does it work?

This project aims at managing patients with chronic conditions safely from the comfort of their own home by implementing digital technology in healthcare delivery, using real-time data from our PPG-technology. This approach leads to care that is more accessible, efficient, and tailored to individual needs. Moreover, it has the capacity to simplify care processes, reduce expenses by supporting early discharge and cutting the average length of stay by 4 days per patient, address disparities in healthcare access, and improve monitoring of patients with changing health conditions.

Patients were onboarded via two streams: either as outpatients following a referral from the SDEC (Same Day Emergency Care unit) or cardiology clinic, or while inpatient at the point of hospital discharge.

The Chelsea and Westminster Hospital is one of many UK centers working with Patients Know Best (PKB), the UK’s leading personal health record provider. This platform allows users to upload their personal health data, and link data from FibriCheck measurements, providing healthcare provider with a holistic overview of patient’s health.
