Objective
To manage service demand more effectively by implementing FibriCheck as the first line of prescription for patients experiencing palpitations.
Approach
Across 146 participating GP practices, patients with palpitations are triaged more effectively. Each practice has its own FibriCheck code to provide when a patient calls; patients self-check during episodes, giving real-time insight and reassurance, with monitoring data centrally managed by one of three hubs.
Outcome
Rather than waiting weeks for a hospital appointment, patients and their GP get timely access to care from home, sometimes avoiding an unnecessary trip to the emergency department. The pathway reduces unnecessary appointments and diagnostic referrals, decreases wait times and creates capacity for those who need it most.
