Objective
To improve the efficiency of the cardiology department at Rijnstate, a Dutch healthcare provider.
Approach
In the digital care pathway, patients check their own heart rhythm remotely with FibriCheck and complete questionnaires in the Rijnstate patient portal. Nurses monitor the pathway remotely to check the health status of patients.
Outcome
Patients only need to come in when necessary, which frees time for both patients and cardiologists. This decreases pressure on the cardiology department and lets hospital resources be deployed more efficiently, while the portal guides patients through each step of the process.
