FibriCheck partners with Patients Know Best, the UK’s leading personal health record provider

The link between the medically certified FibriCheck app and Patients Know Best is now up and running, allowing users of both solutions to seamlessly share and connect their personal health data. This link helps further comprehensive, person-centred assessment and shared decision making for the next steps in care.

With over 1.5 million users, Patients Know Best is the UK’s leading patient-driven health platform, owing to its integration with the NHS App. Users in the Netherlands are also realising the benefits via the integration with the government’s personal health records infrastructure ‘Persoonlijke gezondheidsomgeving (PGO)’.

Users on both sides of the pond can now share their FibriCheck health data such as measurement results and symptom tracking info, along with other medical data like test results. The platform also enables users to link with other applications and devices such as Strava and Fitbit, bringing a centralised view to all of their health-related data. Where connected with provider systems, the platform partnership will support better outcomes for both patients and the healthcare providers that support them by enabling patient-reported data and clinical data to be looked at holistically.

Supporting virtual wards and alignment to usual care pathways

Chelsea and Westminster Hospital NHS Foundation Trust is one of many UK centres working with Patients Know Best (PKB) to empower patients to be more engaged in their health.

Having recognised the value in both FibriCheck and PKB in practice, the Trust wanted to maximise the potential of both solutions in order to transform the way they follow up on their patients. Chelsea and Westminster Hospital NHS Foundation Trust is implementing a Same Day Emergency Care pathway using the FibriCheck and Patients Know Best integration to enable remote monitoring of AF patients. The pathway aims to reduce the number of hospitalisations and beds taken by patients on rate control who present at the hospital with AF or symptoms that are indicative of AF.

Where before these patients were admitted to the hospital for 2 to 3 days for observation or managed on an ambulatory pathway requiring them to attend hospital, they can now be followed up remotely. Patients are asked to monitor their heart rhythm from the comfort of their home for 7 days via the FibriCheck app. Results of these measurements are automatically uploaded to their PKB record where digital nurses at Chelsea and Westminster Hospital NHS Foundation Trust can follow up on these results and reach out to patients in case irregularities or alarm signals show up during the monitoring period.The patients are also provided with a care plan to help them manage their own health. After the remote monitoring period, the cardiologist decides if next steps in care are required for the patient.

To add FibriCheck to your Patients Know Best account, simply follow these instructions:

  1. In the FibriCheck application, go to “Settings”and tap on “Connected apps”.
  2. Next, tap on “Patients Know Best”. Agree to share the FibriCheck measurement data with Patients Know Best.
  3. Connect with Patients Know Best.
  4. Fill in your Patients Know Best email address and password and approve.
  5. Once the connection has been made, tap on “Back to app” to take the first measurement.
  6. Click on “Open” to go to the FibriCheck app.
